Vibrator problem (HTC One M9)

My new M9 (unrooted) is behaving rather strangely. It refuses to vibrate. At first, I thought it was just a broken vibrator thingy. However, when I reboot it, it vibrates on startup, just like it should. The "double tap to wake" feature also causes it to vibrate, and it seems to work just fine when there's an incoming call. However, it completely ignores notifications, including text messages and alarms, no app I've downloaded can cause the damn thing to vibrate, and when I tried to test it by dialing "*#*#3424#*#*" and running the vibration test, it didn't work at all either.
Possibly unrelated, but the Permission Manager app has been crashing on startup lately. It used to work fine - well, it didn't seem to do anything at all, but at least it didn't crash.
Thought I'd ask around here before I try a factory reset.
